2013 Exhibitor Application Information

Applications for exhibitor space and food vendor space at the 2013 Folsom Street Fair^® and the Up Your Alley Fair^® are available by following the instructions below.

The instructions are divided by event. Please note that for the Folsom Street Fair we have both exhibitors and food vendors, while at the Up Your Alley Fair we only have exhibitors. The application process is different for each of these booth types.

The forms provided have been converted to PDF Forms. Please fill them out in your PDF Reader, then print the completed form, sign it if required, and submit your application via fax or mail. Using this process will reduce questions raised due to illegible handwriting.

Exhibitor Application Instructions

To apply for exhibitor space at the 2013 Folsom Street Fair follow steps 1 through 5 in the list below:

[1]	Download the application forms:
	Get booth space application form	Required
	Get liability insurance application form	Optional
	Get the Folsom Street Fair Program Rate Card	Optional

[2]	Complete booth application and return it to the address on the form along with payment and the required documentation.	Required

[3]	Proof of liability insurance is mandatory for all exhibitors. Folsom Street Events must be named as being additionally insured through your carrier and be issued a "Certificate of Insurance."	Required
	If you do not have a liability insurance policy, complete the liability insurance application form above and send it to Casswood Insurance.	Optional

[4]	Rent booth structure	Optional
	Booth structure rental is available for $150.00 per 10'x10' booth by filling out the appropriate section of the application above.

[5]	Place an advert in the official 2013 Folsom Street Fair Guide.	Optional
